2. Complete and submit your claim packet
You may submit your completed forms by email, mail or fax. Once we recieve the completed
claims packet, an examiner will be assigned and begin the claim evaluation process.

3. Receive payment
Once all claims requirements are received in good order, your benefit payment will be processed. You will be able to decide how you would like to receive your payment through a variety of payment options.
